Box 9363–00100, Nairobi in the Republic of Kenya, formerly known as Joyce Aluoch Okoth, formally and absolutely renounced and abandoned the use of her former name Joyce Aluoch Okoth, and in lieu thereof assumed and adopted the name Alexis Aluoch Okoth, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Alexis Aluoch Okoth only.
