Box 29888–00202 Nairobi in the Republic of Kenya, formerly known as Dorcas Wanjiru Kariuki, formally and absolutely renounced and abandoned the use of my former name Dorcas Wanjiru Kariuki, and in lieu thereof assumed and adopted the name Melissa Wanjiru Kariuki, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Melissa Wanjiru Kariuki only.
