Box 7855-00300, Nairobi in the Republic of Kenya, formerly known as Timothy Samuel Wainaina Ngugi, formally and absolutely renounced and abandoned the use of his former name Timothy Samuel Wainaina Ngugi and in lieu thereof assumed and adopted the name Timothy Samuel Wainaina Ngugi Uncle Sam, for ali purposes and authorizes and requests ail persons at all times to designate, describe and address him by his assumed name Timothy Samuei Wainaina Ngugi Uncle Sam only.
