Box 19056- 00500, Nairobi in the Republic of Kenya, formerly known as Edina Wavinya Kioko, formally and absolutely renounced and abandoned the use of her former name Edina Wavinya Kioko and in lieu thereof assumed and adopted the name Edina Wavinya Ogutu, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Edina Wavinya Ogutu only.
