Box 84659-80100, Mombasa in the Republic of Kenya, a resident of Apartment 652, the Iveagh Trust, New Bride Street, Dublin, Ireland, on behalf of Ruqayyah Khalid Hamid El-Busaidy (minor), formerly known as Rugayyah Mohamed Abdi, formally and absolutely renounced and abandoned the use of her former name Ruqayyah Mohamed Abdi, and in lieu thereof assumed and adopted the name Rugayyah Khalid Hamid El-Busaidy, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Ruqayyah Khalid Hamid El-Busaidy only.
