Box 100928- 00101, Nairobi in the Republic of Kenya, formerly known as Alex Muthomi Negatia, formally and absolutely renounced and abandoned the use of my former name Alex Muthomi Negatia, and in lieu thereof assumed and adopted the name Alex Muthomi Kariuki, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Alex Muthomi Kariuki only.
