Box 46750— 00100, Nairobi in the Republic of Kenya, the mother and legal guardian of Ashley Nana Luchinga, (a minor), formerly known as Aisha Nana Buhari, formally and absolutely renounced and abandoned the use of her former name Aisha Nana Buhari and in lieu thereof assumed and adopted the name Ashley Nana Luchinga, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Ashley Nana Luchinga only.
